Dear Applicants,

We welcome and appreciate your application to Heritage Preparatory School. The admission procedure for your child will include the completion of the following items:

1. Make an appointment to have the applicant tested before registration.

2. Schedule an appointment for a personal tour of our campus.

3. Obtain an application package from the registrar's office.

4. Turn in the following completed forms to the registrar's office:

- Application for Enrollment Form (Click here to Download)

- Child Care Registration (if applicable)

5. Remit all registration fees to the receptionist's office.

6. Turn in the following to the receptionist:

(Please be sure to clearly label these forms with student's name and grade.)

• Updated Physical Form (HRS-3040) from the State of Florida
• Up to date Immunization Record (HRS-680) from the State of Florida
• Copy of Official Birth Certificate
• Copy of Applicant's Last Report Card (Grades 1-12)
• Copy of Progress Reports or Testing (if available)
• Signed HPS Release Form for Photographs or Video of Your Child
• Signed Application Which Includes Your Permission for Your Child to Be Transported and Receive Medical Care in an Emergency
• Signed Statement Stating You have Received a Copy of the HPS Parent
• Handbook, VPK Parents Also Sign for a Second VPK Handbook
